If you are brand new to us, choose the NEW PATIENT option. If you were in an accident choose the NEW PERSONAL INJURY option.

All of our online appointments are considered PENDING until you receive a text or call confirmation from us. We reserve the right to adjust your appointment times based on our provider's availability.  Please fill out your paperwork immediately, it will come to your email once you book.

*Our Elite and Premium Treatments have distance restrictions.  

 

Deposit Policy

1. Deposit Requirement for all NEW Patient appointments

A deposit is required to secure all new appointments. The deposit amount will be applied toward your service of choice.

2. Refund and Cancellation Policy

  • Cancellation and Rescheduling: Clients may cancel or reschedule their appointments without penalty if notice is given at least 4 hours before the scheduled appointment time.
  • Refunds: Deposits will be refunded in full if the appointment is canceled or rescheduled with at least 4 hours notice. If the cancellation or rescheduling is made less than 4 hours before the appointment time, the deposit will be forfeited.
  • No-Show Policy: Clients who do not show up for their appointment without any prior notice will forfeit their deposit.

3. How to Cancel or Reschedule

To cancel or reschedule your appointment, please contact us via phone or text at 480-442-1301 or email info@housecallchiros.com at least 4 hours before your scheduled time. Alternatively, you may use our online booking system to make changes to your appointment.
